Supported Living Arrangements (SLA)
Program Description
The Patricia Lloyd Townhomes located in Evanston and scattered-site apartments located in Evanston and Skokie comprise SHORE’s Supported Living Arrangements (SLA) program. The Patricia Lloyd Townhomes are a row of five two-story units, each housing three individuals that are available to adults who are able to live more independently. Twelve individuals live in the homes and most are employed in the community. Residents are responsible for themselves and their homes. House Managers reside in one unit to provide support as needed. The scattered-site apartment residents have intermittent contact with staff. Services are provided on an as-needed basis and include: budgeting, housekeeping training, interpersonal counseling, case management, cooking, shopping, transportation training, etc.
Eligibility Requirements
- Individuals with a developmental disability, 18 years of age and older, functioning in the mild range and not dangerous to themselves or others.
- Capable of self-medication, attested to by a physician.
- Ability to care for personal and housekeeping needs with a minimum of supervision.
- Not requiring frequent on-site medical care, and not having a severe or chronic mental illness or addiction to drugs or alcohol.
- Employment in a competitive or sheltered work setting or participation in a vocational
training program or volunteer activities.
- Recent application filed for social security benefits or receipt of such benefits, if not gainfully employed.

Intake Process
- Initial tour of SLA
- Completion of Intake Packet
- Intake Interview
- Submission of admission information; i.e., psychological, medical reports, social history
- Intake Committee review
