Program Description
SHORE Homes consists of two twelve-bed residential facilities with 24-hour staff supervision. SHORE Homes East-Buehler House, located in Evanston, is an Intermediate Care Facility for the Developmentally Disabled (ICF-DD), and SHORE Homes West, located in Skokie, is a Community Living Facility (CLF).
Although 24-hour staff is present, the goal is to provide an environment as similar as possible to everyday life in mainstream society. Interaction with the community and a focus on becoming less dependent on others is encouraged. The homes are residential facilities with the objective being to move residents to a more independent living environment.
Eligibility Requirements
- Individuals with developmental disabilities, 18 years of age and older, functioning in the mild to severe range and not dangerous to themselves or others.
- Sufficient medical stability to live in a group setting without frequent on-site medical care, and capable of self-preservation in an emergency.
- Recent application filed for social security benefits or receipt of such benefits.
- Employed in a competitive or sheltered work setting or enrollment in an external day program.

Intake Process
- Initial tour of residential home
- Completion of Intake Packet
- Intake Interview
- Submission of admission information; i.e., psychological, medical reports, social history
- Intake Committee review
