Service Coordination
Program Description
Service Coordination is provided for those individuals who are enrolled in the Illinois Home Base Support Services program. The program is designed to help individuals with developmental disabilities maintain community placement within their family home. Case management and support is provided by SHORE’s Service Facilitator for the individuals and their families including: identifying and assisting linkage to identified service needs for the person served.
Eligibility Requirement
- Individuals with developmental disabilities, 18 years of age and older.
- Resident of Illinois
- Eligible for Medicaid

Intake Process
- The PAS Agency will contact SHORE, sending a referral packet.
- Family then contacts Service Facilitator.
- Service Facilitator conducts a home visit and completes paper work.
