CILAs (Community Integrated Living Arrangements)

Program Description

SHORE’s Community Integrated Living Arrangements (CILAs) consist of three sites; a home in Evanston for two men, a home in Skokie for four men, and a home in Skokie for six women. Twenty-four hour staff supervision is provided in each home to help individuals with specialized needs become as independent as they are able. The goal in the CILAs is to deliver services, which provide for the total array of mental health and developmental service needs to support and assure integrated community living.

 

Eligibility Requirements

  • Individuals with a developmental disability, 18 years of age or older
  • Eligible for SSI/SSDI and Medicaid
  • Substantial limitations in three or more of the major life activities; i.e., self-care, language, learning, mobility, self-direction and capacity for independent living.
  • Ability to maintain or stabilize behavior so it is not a danger to themselves or others.
  • Willingness to participate in meaningful day activities.

Intake Process

  • Initial tour of the CILA
  • Completion of Intake Packet
  • Intake Interview
  • Submission of admission information; i.e., psychological, medical reports, social history
  • Intake Committee review
